Home Health Aides:

DUTIES AND RESPONSIBILITIES:

The Personal Care Aide is expected to perform the duties assigned by the Passport Case Manager and any special instructions prepared by the Registered Nurse at each assessment.


Personal care duties include the following:

Bathing- bed, tub, shower; complete; partial

Skin care

Hand and foot care

Applying cosmetics

Oral hygiene

Hair care

Shaving

Perineal care

Reporting suspected abuse, neglect, and exploitation


Mobility Care includes the following:

Proper body mechanics

Transfer from bed to chair

Assist ambulation using transfer belt, wheelchair or walker

Passive range of motion exercises of normal joints

Turning and positioning

Ambulation


Elimination duties include the following:

Assist with use of bedpan, bedside commode, toileting activity

Incontinent care


Nutrition care include the following:

Meal planning and preparation

Special diet preparation with instruction

Cleaning of eating and food preparation areas

Feeding

Encouraging and facilitating adequate fluid intake


Other duties may include:

Grocery shopping

Errands for prescriptions or other medical items

Reality orientation



QUALIFICATION REQUIREMENTS: 

State tested certification or 60-hour training course.

High School diploma or GED.

Ability to get along with senior citizens.

Pass the criminal background check.

Valid Ohio Driver’s license.

Must have reliable transportation service, a car available for work, and vehicle insurance that meets the Financial Responsibility Law requirements.  The Ohio Revised Code Section 4509.101 requires that all operators and owners of motor vehicles driven upon roads in Ohio must maintain proof of financial responsibility.


To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill, and or ability required.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.


Education and/or Experience:  High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.


Language Skills:  Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively before groups of customers or employees of organization.


Mathematical Skills:  Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.


Reasoning Ability:  Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.  Ability to deal with problems involving several concrete variables in standardized situations.


Other:  Each person must have a one-step Mantoux prior to employment.


Certificates, Licenses, Registrations:  Must have valid Ohio Driver’s License with clean driving record which meets the company’s fleet insurance company standards:  (a) an Ohio MVR report containing 2 years’ driving experience which will be reviewed and (b) anything over four points –or- four points or four points that pertain to reckless operations or DUI are considered unacceptable.   Must have reliable transportation service, a car available for work, and vehicle insurance, which meets the Financial Responsibility Law requirements.  The Ohio Revised Code Section 4509.101 requires that all operators and owners of motor vehicles driven upon roads in Ohio must maintain proof of financial responsibility.

While performing the duties of this job, the employee is regularly required to use hands to finger,

handle or feel objects, tools, or controls; talk or hear; and taste or smell. 


The employee frequently is required to stand; walk; sit; reach with hands and arms; and stoop,      

kneel, crouch, or crawl.  The employee is occasionally required to climb or balance. 


The employee must lift and/or move up to 10 pounds, frequently lift and/or move up to 50 pounds,

and occasionally lift and/or move up to 100 pounds.  Specific vision abilities required by this job

include close vision, distance vision, color vision, peripheral vision, depth perception, and the

ability to adjust focus.

Transportation Aides:


  1. SUMMARY:  Drives automobile or van to pick up and deliver Everyday Homecare clients to approved locations and maintains records of following duties:


ESSENTIAL DUTIES AND RESPONSIBILITIES 

  1. -Other duties may be assigned. 

  2. -Drives automobile or van to transport clients to and from approved locations.

  3. -Files an incident report for any accidents, injuries, etc., that might occur.

  4. -Records information concerning clients transported; mileage, gasoline, oil, repairs/maintenance needed.

  5. -Insures that vehicle is in proper operating condition (i.e., oil/oil & air filters, tire pressure, operating fluids, etc).  Completes checklist before beginning each trip.

  6. -Insures that all appropriate safety procedures are followed concerning clients and operation of the vehicle.

  7. -Listens to and resolves service complaints.  Makes referrals to appropriate person(s) for unresolved issues.

  8. -Abides by driving laws in whatever jurisdiction in which you travel (i.e., speed, seat belts, signals, clearance, etc.

  9. -Cleans inside and outside of vehicle.  Performs routine maintenance on vehicle.

  10. -All records are to be maintained in strict confidence.

  11. -Perform other duties of short duration from time to time as required by supervisor.

  12. -Reporting suspected abuse, neglect, and exploitation


ALL TRANSPORTATION AIDES WILL MEET THE FOLLOWING TRAINING REQUIREMENTS:


1.  Completion of a passenger assistance course meeting the minimum criteria as specified by passport, and must complete the training within the six months prior to hire or within the first year of employment by the provider.

2.  Completion of one of the following defensive driving programs within one year prior to hire or within the three months of employment as a vehicle operator by the provider.

•National Safety Council

•Ohio Department of Development

•Any other defensive driving course approved by Passport.

3.  Completion of an ODA or PAA approved aging sensitivity course within six months of employment by the provider.

4.  Completion of a training course in first aid and current cardiopulmonary resuscitation (CPR) certification within six months of employment.



QUALIFICATION REQUIREMENTS: 


Have at least two years of licensed driving experience.


Have received instruction in transfer techniques prior to transporting enrollees.
